Youth Transitions Initiative

The HSC Foundation has launched a Youth Transitions Initiative, designed to become a long-range signature project to assist young people with disabilities and chronic illness in the Washington metropolitan area and beyond to move from school to adulthood to the world of work.

Family Supports

The HSC Foundation is committed to supporting families and other individuals who are providing care for infants, youth, and adults with chronic illness, disabilities, at-risk for disabilities, or who have unique health care needs.

Childhood Obesity Initiative

The HSC Foundation has adopted an initiative that aims to analyze qualitative data and formulate recommendations that will inform government officials, funders, and other stakeholders on effective education and communications strategies, policy opportunities and obstacles, and the private sector’s role in supporting education and policy change.

Awards Programs

The HSC Foundation operates two awards programs—one designed to enrich the lives of youth and young adults with disabilities and chronic illnesses in the Washington metropolitan area, and one that recognizes individuals and nonprofit organizations that were effectively working to positively impact the lives of people with disabilities.
